Traditional patient and elderly care protocols have changed due to the advancement of technology, prolongation of life expectancy, increase in chronic diseases and health expenditures, increase of individuals in need of care, increase of individuals' expectations. SCOPE OF HOME HEALTH SERVICES AND SERVICES PROVIDED:
It is not the duty of home health services to diagnose illness. Within the scope of diagnosis and planned treatment previously diagnosed by the relevant specialist; providing examination, examination, analysis, treatment, medical care and rehabilitation services in the home environment, assisting in the preparation of reports on the use of medicines, medical devices and materials, informing the patient and his / her family about the tasks they can undertake in the home care process and the disease and care processes and the provision of training and consultancy services on the proper and proper use of medical devices and equipment requiring domestic use.
PATIENT VISITS:
Home health services are carried out by appointment system. According to the condition of the patient and the disease; visits are planned daily, weekly or monthly. The frequency of visits is determined by the physician in accordance with the care and treatment plan. In accordance with the determined visit plan, services are carried out by appointment system. In non-urgent cases, an appointment can be requested with the patient's request, and planning is made if the physician deems appropriate.
HOW TO APPLY:
Patients, relatives, hospital staff or family physicians; For the patient who is deemed eligible to receive home health service, the Ministry applies by calling the Home Health Services call center on 444 38 33 . The call center receives the patient's identification information and directs it to the provincial coordination center. The coordination center calls the patient within 15 minutes and receives the necessary health information. Refer to the relevant home health unit. The first visit of the patient is made within 24 hours by the health department of the relevant home. Information is given to the patient within the framework of the treatment or other home health services that the doctor deems appropriate. The treatment plan of the patient is created or terminated.
